How to take the Survey
One survey completed represents each individual's unique life.
Our goal is to get one completed survey for as many A/I/DD individuals as possible in our community.
The more voices, the more choices.
Before you start your survey, it will be helpful to use these worksheets and the resources below to make it easier to think about the many considerations covered in the survey.
SURVEY PROCESS

The Standard Learning session and survey are designed for individuals with A/I/DD, family members and adults who support these individuals. Standard Surveys focus on important life considerations, and the 50 questions are designed to be completed by individuals with A/I/DD in partnership with those who support them. This is the preferred survey because it offers more information.
The Simplified learning session and survey are designed in an easier-to-understand format for individuals with A/I/DD. Because the goal is to have one survey per A/I/DD individual, we are asking family members and adults who support these individuals to take the Standard Survey in partnership with their A/I/DD individual if possible. The Simplified Survey is designed for self-advocates to complete if they can't complete the Standard Survey on their own or with the help of adults who support them.

Guides, Slides, Videos, and Worksheets are available for reference to make it easier to complete surveys.
3. Take Survey
Once complete, you can print and/or save a copy of your confidential responses to use as a tool in continued person-centered planning.
4. Share Experience
Encourage others to participate! The more survey responses shared, the greater the input will be for achieving the best housing options possible.
